Weight-Management Medication — Patient Care & Medication Services Agreement
1. Eligibility
- Medication may only be supplied following receipt of a valid prescription from a registered UK prescriber.
- The prescriber must verify and record the patient’s BMI in the prescription questionnaire along with other requested information.
- If BMI is 27–29.9 kg/m², at least one qualifying comorbidity must also be recorded.
2. Prescription Requirements
- Prescriptions missing BMI, comorbidity (when required), or appropriate clinical details will be rejected. The questionnaire must be completed in full.
- Suspected adverse reactions should be reported via the MHRA Yellow Card scheme. This can be done by the patient or prescriber.
3. Limits on Supply
- A maximum of 2 boxes of weight-management medication per patient per two months may be dispensed.
4. Ordering Conditions
- Weight-management medication must be ordered separately from all other items.
- Checkout cannot be completed for additional products until the weight-management prescription has been processed.
5. Delivery
- Weight-management medication will be shipped directly to the patient.
- The prescriber is responsible for providing an accurate patient address (property number, first line, town, postcode) and contact number.
- Orders where delivery details do not match the patient information will be flagged/queried and may be rejected.
6. Product Classification
- Weight-management medications are prescription-only and can only be dispensed in accordance with a valid clinically appropriate prescription.
7. Acceptance of Terms
- Fox Pharma reserves the right to query, reject, or cancel prescriptions or orders that do not meet these terms.
