Who is the ideal candidate for under-eye rejuvenation/tear trough treatment? 

“The idea that a practitioner can look at a face and know an ideal candidate for tear trough filler is a little misleading. It requires a clinical assessment to establish the “ideal” candidate and even then, the suitability comes down to the skill of the practitioner. Three clinical presentations always remain true for a patient to have higher odds for a successful treatment: good skin quality, no on-going oedema and true hollowing present.” 

“The presence of all three of those things could constitute the ideal tear trough candidate but even at that, it presents differently on different patients and also requires a suitable medical history.” 

As an expert in this field of work, what is your preferred technique to use when treating the under eyes? 

“I think it’s safe to say that hyaluronic acid (HA) fillers are the gold standard for managing the infraorbital region. They have a level of reliability; they are immediate and still are one of the few injectables options that can add good volume to the region. Now, of course, HA fillers come with their risks and so regenerative options are absolutely making a push as a combination treatment or even a replacement in some cases. I use both at a 1:1 ratio in my practice. Irrespective of the modality chosen, using a cannula in this region is a necessary skill. I suggest that all injectors commit to being entirely comfortable feeling the plane depth when using a cannula. When you have become more advanced, then combining cannula and needle work yields the most optimal results. Again, only once injectors are very advanced.” 

What is your top piece of advice for practitioners treating the under-eye area? 

“Make sure you get educated. This area is one of the most in-demand areas to treat and brings ample satisfaction to patients and practitioners when done correctly but make no mistakes; it’s a toughie! Ensuring you’re staying up to date with your education around techniques and products is crucial. Probably nowhere in the face does the technique-product combination matter more. Beyond that, it’s about getting the repetitions in. You cannot be good at what you don’t practice and as long as injectors are being safe, they should expect linear progression in their ability to treat the region.”
